Client Session Questionnaire

In preparation for your session, please complete this questionnaire within seven days to secure your appointment.  Remember, these images are all about YOU!  Please answer these these questions as accurately as you can, sharing as much information as you are willing to disclose.  Your answers are 100% confidential and will not be shared with anyone else.

    By completing this form, you are agreeing to the following portraiture agreement:

    Portrait Agreement: The session fee including 7.125% sales tax ($160.69) covers a 60-90 minute session at Molly’s studio or a previously agreed upon location, Post-session custom image preparation with attention to each image, and 20-40 images presented to you in a private online gallery. If you are booking a newborn session, the session fee of $200 plus 7.125% tax is $214.25. In order to hold your session time, your session fee plus tax ($160.69 or $214.25) and minimum order  ($200) must be received within seven days of submitting this agreement.  Please make out two separate checks. The session fee and minimum order are  non-refundable but Molly will gladly reschedule due to illness or other unforeseen circumstances; please notify Molly at least 48 hours before your session if you must reschedule. Your deposit can be applied to your session at a later date.  If the weather isn’t agreeable on the day of the session, we will reschedule to another date. If there is a chance of rain in the forecast (for an on location outdoor shoot), Molly will notify you a day or two before the session. Please note that the use of cameras (still and video) other than the photographer’s is prohibited during your session. Images will be available approximately 2-3 weeks after your session and will be presented to you in a private online slide show that you are encouraged to share with friends and family.  A word about copyright: All images taken at your session will be copyrighted by Molly Shields and are protected by Federal Copyright Law.  Moreover, Molly holds the right to use images from your session on her online portfolio, blog, as well as advertising and entering into photographic contests. By checking the following box you are stating that you have read and understood the information in this agreement.
